Was there adequate information noted in the task to support the outcome? If the TOU had conflicting information or was unclear, was justification for the outcome and actions (i.e. written permission requested or not) added to the comments?
Rationale for outcomes, especially in the case of complicated or unclear TOUs, is needed in the event a review of the request is needed in the future.
If the Outcome was not allowed, did the reviewer add the X before the title?
If an alternative website was offered and accepted, was the ID told to create a new request for the alternative?
